4. Free Video Consultation:

5. Additional Medical Notes/History:

If applicable, any additional medical notes or history can be sent to our CBSL team at this stage. This information can further inform the development of your support plan.

6. PBS Care Plan Development:

Based on the assessment findings and collaborative discussions, our CBSL team will begin developing your personalized Positive Behavior Support (PBS) care plan or PBS grab sheet. This plan will be meticulously tailored to your unique needs.

7. Video Call and PBS Plan Delivery:

A video call will be arranged with you to discuss and deliver your PBS plan. This allows for detailed explanations and ensures you have a clear understanding of the strategies and interventions.

8. Four-Week Follow-Up:

After four weeks from the start of PBS plan implementation, a follow-up video call will be scheduled to assess progress and address any questions or concerns.

9. 12-Week Review/Evaluation:

A 12-week review and evaluation video call will provide an opportunity to assess the effectiveness of the PBS plan and make any necessary adjustments.

10. 6-Month Review/Evaluation:

A 6-month review and evaluation video call will ensure the continued success of the plan and make refinements if needed.
